CDC Dog Import Form
The free online form every dog entering the United States needs since August 2024. The receipt is valid 6 months for multiple entries, and airlines check it at boarding.
Since 1 August 2024 the United States asks for it for every dog. You fill it online before travelling, it costs nothing, and you get a receipt with a QR code tied to one dog and one owner. The airline checks that receipt at check-in, so carry it with you, printed or on your phone, not in the checked luggage. It is only one of the universal requirements: the dog must also be at least 6 months old, with no exceptions by country or airline, carry an ISO-compatible microchip implanted before any required rabies vaccination, and look healthy on arrival. If the dog has been in a country the CDC lists as high risk for dog rabies in the previous 6 months, the form is not enough on its own.
